At your first visit we ask you to bring your health-insurance card and ID, so that we can finish preparing your patient record on the spot.
Tandartspraktijk Voorschoten uses a limited registration form on this website, because of the General Data Protection Regulation (GDPR) that has been in force since 25 May 2018.
